Note 1: Attach a copy of the identity document, academic diploma or degree, and minimum 90-hour specialized training certificate, according to the applicable certification scheme.
Note 2: The certification service is a voluntary process carried out by the applicant and arranged with the Personnel Certification Body, in which the applicant must demonstrate compliance with Labor Competence Standards.
Note 3: The organization guarantees the confidentiality of the information provided.
Note 4: Each type of certification is independent and requires specific evidence.
